The reprinting of this extremely rare text was initiated by Robert Toll.  The Popuar Press edition provided a short introduction by Toll..  The original is assumed to have been self-published, in Chicago.  At the time you could have purchased a copy at 436 Dearborn St. by addressing Ike Simond.  The 62 page chapbook cost the buyer 25 cents.
